The next person on shift needs the full picture.
When daily care involves a feeding routine, catheter care or several clinical tasks, it takes more than a general list of services to plan the right support. We go through your current care information and talk about what a usual day and night involve.
Nurses and support workers have different responsibilities. We identify the skills each task requires and how the people involved will follow your individual care plan.
We also discuss handovers: what the next worker needs to know, how changes are recorded and who should be contacted when something is different. These details are part of planning your support from the start.
Planning overnight support.
The right arrangement starts with what you actually need during the night.
Sleepover support
A worker sleeps at the home and responds when assistance is needed, within the agreed arrangement.
Active overnight support
A worker stays awake to provide support throughout normal sleeping hours.
Overnight nursing
Nursing care is available at night. We discuss the clinical tasks and how nursing fits into your care plan.

A clear handover between the evening and overnight team.
A participant has a tube-feeding routine and needs assistance during the night. Their family wants to understand which tasks require a nurse and what information the next worker needs.
KIRAYA would review the care plan and the person’s usual night-time needs to discuss the right skills and overnight arrangement.
The plan would set out who provides each task, what is included in the handover and who to contact when needs change.
Everyone understands their part.
The aim is for day and night support to follow the same care information, with nursing involvement planned around the participant’s clinical needs.
